Claims Reporting & Nurse Triage

If you sustain an injury on the job and have not yet sought medical attention:

If you experienced an urgent or emergency medical situation on the job and already received medical treatment:

If you are an employer and one of your employees sustains a minor injury that does not require a call with the triage nurse:
Please fill out an Incident Report Form and keep it on file for your records.

Reporting Requirements
- All work-related injuries should be reported as soon as possible, but no later than 10 days after the date of injury.
- Time-loss claims should be reported within 24 hours.
- Deaths must be reported immediately to Securis via telephone at 888.603.0144, and to the Industrial Commission of Arizona (ICA) at 602.542.1839.
- Employers are required to report the following to OSHA:
- all work-related fatalities
- all work-related inpatient hospitalizations of one or more employees that occur within 24 hours of a work-related incident
- all work-related amputations
- all work-related losses of an eye
- Work-related fatalities must be reported within eight hours of learning of them. Work-related inpatient hospitalizations of one or more employees; amputations; or losses of an eye that occur within 24 hours of a work-related incident must be reported within 24 hours of learning of them.
